I have been using a TaylorMade Spider counter balance putter that is about 34 in in length. Is this 38 in putter only for taller players or is the longer length part of how it functions ?

You can "subtract" 3 inches to determine the proper length for standard use. The way to use the dual balance putter is to grip down on the handle to put the additional weight above your hands. For me, I use a 34 inch putter, so I had my dual balance putter cut down by one inch (by Scotty Cameron) to 37 inches. In summary, this putter is NOT for taller players. Treat it as a standard putter with an extra 3 inches for dual balance purposes. If you need a longer putter, then you will have to custom order a shaft for the added length. If your standard putter is 36 inches, for example, then you would need a 39 inch dual balance putter. Hope that helps.

can i change the weights in the bottom - and can it be ordered with the heavier weights already installed

You can change out the weights in the bottom of this putter using a weight kit that is offered through Titleist. Unfortunately, these putters do not come with a higher weight already installed.
